clients in identifying appropriate long-term care and housing options. Personal information is

requested to understand a client’s care needs, preferences, and eligibility factors relevant to

placement decisions.

Personal information is collected lawfully and fairly, with the client’s knowledge and consent.

Information is collected directly from the client or the client’s authorized representative through

conversations, written forms, or electronic communication. We collect only the information

necessary to provide referral services.

Consent and Disclosure

Before sharing any personal information, Path Forward Senior Transitions LLC provides each client

with a Disclosure and Advisory Form and this Privacy Policy. We obtain the client’s clear,

affirmative consent prior to sharing personal information with any facility. Information will only be

shared after consent has been obtained.

Information Sharing

Client information is shared only with long-term care providers or related services to which the client

has agreed to be referred. We do not sell, trade, or share personal information with marketing

companies or unrelated third parties.

Accuracy

Clients and authorized representatives are responsible for providing information that is accurate,

complete, and current. Clients may request updates or corrections to their personal information at

any time.

Security Measures

Path Forward Senior Transitions LLC takes reasonable measures to protect personal information

from loss, theft, unauthorized access, use, or disclosure. These measures include secure storage

of records, password-protected devices and accounts, and limiting access to authorized personnel.

Client Requests

Clients and authorized representatives may request to review, update, or correct their personal

Record Retention

Path Forward Senior Transitions LLC retains each client’s signed Disclosure and Advisory Form for

three (3) years, in accordance with Oregon guidelines. Sensitive information, such as detailed

health or financial information, is not retained longer than necessary to provide referral services.
